Based on the evidence obtained during the DFPS investigation there is sufficient evidence to confirm that a caregiver did not utilize good judgement as they grabbed/pulled a child by the arm. A caregiver pushed a child once causing the child to bump into a nearby shelf, twice causing him to fall on his buttocks on the floor. The caregiver also slung the child by the arm, and the child fell forward onto what appeared to be a cushioned mat.

After management reviewed video footage and identified additional events that may have contributed to the child?s injury, the incident report did not initially reflect those findings and required revision.
