During the inspection, children were observed rough playing, one child was observed punching another child in the private area, another child was observed holding a child against the wall and holding their hand around the child's neck. The child was in against the wall for approximately 20-30 seconds before a caregiver came to intervene. Children were also observed running back and forth between two rooms, sitting and climbing on furniture, with little to no intervention from the caregivers present.

The caregivers were not aware of the ages of children present. One caregiver stated they do not go by ages but by grade level and that the children should be between 5-11 or 12yrs old. A second caregiver stated they did not know because they were a substitute. There was only information available for the children from one school. According to the caregiver in charge, the driver had the other rosters for a pick-up run.

A total of 21 children were observed including 10 children who were escorted to the restroom. The caregiver was asked to count again to verify the number of children in their care. The caregiver then counted and stated there were 10 children in their care and remainder went to the restroom giving the group a total of 18.
