A video tape was reviewed of 9/29/2021 incident. There were fourteen 3yo-5yo observed in care. A child was observed left in the classroom bathroom unattended and in the classroom for 2 minutes. The caregivers had taken thirteen children outside to the playground without the child while in the bathroom.
