      Based on observation, the licensee did not comply with the section cited above in that LPA found an exposed rusted screw and nail on a piece of the wall accessible to children which poses/posed a potential health, safety or personal rights risk to persons in care.

      Lead Testing

      Based on LPA Wilburn's observation, the licensee did not comply with the section cited above in the facility water system has not had lead exceedance testing, which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: LPA Wilburn discussed the requirements that went out, and LPA provided Director with a copy of PIN 21-21-CCP for reference. Principal advised they already have a company that the facility used in the past, and he will reach out to them to schedule the lead testing. Director will submit proof of completion/clearance to LPA by plan of correction date of 10/06/2023.

      Plan of correction: During file review, LPA Wilburn observed a written note in the section for "start date," which said C3 would not be able to start until parent produced a physician's report. Director stated she still has not received the physician's report. Director will obtain a copy of a physician's report and current immunization's and submit a copy to LPA by plan of correction date of 10/06/2023.

The Director's(S1) certificate expired 11/05/2021

      Plan of correction: Director will ensure all staff complete the required training (General Training & Child Provider Care AB 1207) and submit proof to LPA by plan of correction date of 10/31/2022. The training could be completed at www.mandatedreporterca.com

      Plan of correction: Director will ensure that all staff have required immunization's (MMR, Pertussis & Influenza). Staff are allowed to decline the Flu Shot, but they must complete a waiver statement. Also, staff are allowed to do blood work, to confirm immunity from Measles, Mumps, Rubella and Pertussis, if they do not have a record of their immunization's. Director must submit proof to LPA by plan of correction date of 10/31/2022
