Lead Testing

Based on LPA Wilburn's observation, the licensee did not comply with the section cited above in the facility water system has not had lead exceedance testing, which poses/posed a potential health, safety or personal rights risk to persons in care.

Plan of correction: LPA Wilburn discussed the requirements that went out, and LPA provided Director with a copy of PIN 21-21-CCP for reference. Principal advised they already have a company that the facility used in the past, and he will reach out to them to schedule the lead testing. Director will submit proof of completion/clearance to LPA by plan of correction date of 10/06/2023.

General Provisions and Definitions

Based on LPA Wilburn's record review, the licensee did not comply with the section cited above in 1 out of 4 Staff, Staff #2 (S2) did not have proof of required immunization's available for review at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.

Plan of correction: Director will submit copies of Staff #2's required immunization's to LPA by plan of correction date of 10/06/2023.

Personnel Requirements

Based on LPA Wilburn's record review, the licensee did not comply with the section cited above in 3 out of 4 Staff, Staff #1(S1), Staff #2 (S2) and Staff #4 (S4) do not have proof of a health screening and S2 does not have proof of a health screening and TB test with results available for review at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.

Plan of correction: Director will ensure that S1, S2, and S4 have all required paperwork, and she will submit a copy to LPA by plan of correction date of 10/06/2023.

Teacher Qualifications and Duties

Based on LPA Wilburn's record review, the licensee did not comply with the section cited above in 2 out of 4 Staff, Staff #3 (S3) and Staff #4 (S4) did not have proof of educational requirements/transcripts available for review at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.

Plan of correction: Director will submit proof of transcripts or enrollment for Staff #3 and Staff #4 by plan of correction date of 10/06/2023.

Child's Medical Assessments

Based on LPA Wilburn's record review, the licensee did not comply with the section cited above in 1 out of 6 children, Child #3 (C3) did not have proof of a physician's report at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.

Plan of correction: During file review, LPA Wilburn observed a written note in the section for "start date," which said C3 would not be able to start until parent produced a physician's report. Director stated she still has not received the physician's report. Director will obtain a copy of a physician's report and current immunization's and submit a copy to LPA by plan of correction date of 10/06/2023.

Administration of Child Day Care Licensing

Based on record review, LPA Wilburn observed the licensee did not comply with the section cited above in 8 out of 8 Staff, S1 through S8 did not have proof of mandated reporter training certificates, which poses/posed a potential health, safety or personal rights risk to persons in care. The Director's(S1) certificate expired 11/05/2021

Plan of correction: Director will ensure all staff complete the required training (General Training & Child Provider Care AB 1207) and submit proof to LPA by plan of correction date of 10/31/2022. The training could be completed at www.mandatedreporterca.com

Based on record review, LPA Wilburn observed the licensee did not comply with the section cited above in 6 out of 8 Staff, S3, S4, S5, S6, S7 and S8 did not have proof of immunization's at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.

Plan of correction: Director will ensure that all staff have required immunization's (MMR, Pertussis & Influenza). Staff are allowed to decline the Flu Shot, but they must complete a waiver statement. Also, staff are allowed to do blood work, to confirm immunity from Measles, Mumps, Rubella and Pertussis, if they do not have a record of their immunization's. Director must submit proof to LPA by plan of correction date of 10/31/2022

Personnel Requirements

Based on record review, LPA Wilburn observed the licensee did not comply with the section cited above in 6 out of 8 Staff, S2, S3, S4, S5, S6 and S7 did not have proof of immunization's at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.

Plan of correction: Director will ensure all staff obtain current first aid and cpr certification and submit proof to LPA by plan of correction date of 10/31/2022.

Based on record review, LPA Wilburn observed the licensee did not comply with the section cited above in 5 out of 8 Staff, S3, S5, S6, S7 and S8 did not have proof of a health screening and TB test with results at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.

Plan of correction: Licensee will ensure all staff have required health screening and TB test with results and submit proof to LPA by plan of correction date of 10/31/2022.

Based on record review, LPA Wilburn observed the licensee did not comply with the section cited above in 4 out of 8 Teacher Aides, S5, S6, S7 and S8 did not have proof of transcripts to verify units and/or credits in Early Childhood Education or Child Developments at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.
