Per Facility Representative, they will create a form that is like an "orientation" form for new substitutes that will go over important information, including Personal Rights, that new substitutes will sign as acknowledgment. A copy of "orientation form" will be sent to LPA by POC date. Also Options will contact Zen Educate to inform them that they no longer wish to have Staff #2 as a substitute for all Options For Learning locations.
