     Administration of Child Day Care Licensing

     Based on interview and record review, the licensee did not comply with the section cited above in three of seven staff files audited. Where S4 and S5 mandated reporter certificates were expired and S8 certificate was not from an approved vendor. Which poses a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Director agrees to email LPA current Mandated Reporter certificates for S4, S5 and S8.

     Correction due May 22, 2026. Standard 1596.8662(b)(1).

     General Provisions and Definitions

     Based on interview and record review, the licensee did not comply with the section cited above in one of eight staff files audited. Where S7 proof of TDAP was not availalel for review during inspection. Which poses a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Director agrees to email S7 proof of TDAP immunization records or confirmation of antibody testing.

     Correction due May 22, 2026. Standard 1596.7995(a)(1).

     Personnel Requirements

     Based on observation, interview and record review, the licensee did not comply with the section cited above in two of two staff opening shifts. Where S2 and S3 did not have current or approved EMSA CPR certificate on file. Which poses a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Director agrees to email LPA a written plan regarding both opening and closing staff having a current CPR certificate on file. DIR stated they understand CPR and FIrst AId is required every two years and must be obtained through American Red Cross, American Heart Association or an approved EMSA vendor.

     Correction due May 22, 2026. Standard 101216(f).

     Personnel Requirements

     Based on record review, the licensee did not comply with the section cited above in two of four staff files reviewed. Where Staff 3 (S3) was missing proof of TB clearance and S4 did not have a physical availbale for review. Which poses a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Director will email LPA documentation of TB clearance for Staff 3 and a physical for Staff 4. This is a repeat violation from a 5/29/24 visit, generating an automatic civil penalty of $250.
