      Administration of Child Day Care Licensing

      Based on interview and record review, the licensee did not comply with the section cited above in three of seven staff files audited. Where S4 and S5 mandated reporter certificates were expired and S8 certificate was not from an approved vendor. Which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Director agrees to email LPA current Mandated Reporter certificates for S4, S5 and S8.

Where S2 and S3 did not have current or approved EMSA CPR certificate on file. Which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Director agrees to email LPA a written plan regarding both opening and closing staff having a current CPR certificate on file. DIR stated they understand CPR and FIrst AId is required every two years and must be obtained through American Red Cross, American Heart Association or an approved EMSA vendor.

      Based on record review, the licensee did not comply with the section cited above in two of four staff files reviewed. Where Staff 3 (S3) was missing proof of TB clearance and S4 did not have a physical availbale for review. Which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Director will email LPA documentation of TB clearance for Staff 3 and a physical for Staff 4. This is a repeat violation from a 5/29/24 visit, generating an automatic civil penalty of $250.

      Based on observation a perimeter gate in the playground was broken, leaving an opening large enough for a young child to exit to the parking lot of the facility which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Director immediately removed the knife from the classroom and stated knives will not be left in the classroom or in the cabinet.
