Based on observation, the licensee did not comply with the section cited above as LPA observed preschool play structure has no age recommended manufacture label, the rubber that coats the metal bars, handles and stairs are warn off and torn off, spider webs around the play structure, paint peeling on the side. LPA's observed a picnic style bench with lattice style roof that had 4 rusted poles, and a rusted faux faucet. 1 water fountain that has low water pressure which poses a potential health, safety or personal rights risk to persons in care.

DIrector reported they will hire a play ground inspector to assess the preschool play structure by POC due date. The play ground inspector will be able to ensure licensee procurs age appropriate stickers for climbing structure. Director will monitor play structure for cleaniness and safety. DIrector reported they will paint or repair picnic style bench to get rid of rust and make surface smooth. Director reported they will make fountain inaccessible to children by placing a structure over the fountain.

Per Site Supervisor Mateus all authorized representatives have been notified of the lead exceedance and testing results. The two outlets were made inoperable and replaced. Mateus states retesting of the 2 outlets is in progress. Retesting is to be completed by 2/10/2023.
