Based on information reported and interviews conducted the facility did not comply with the section cited above in regards to two children being without supervision.

Signed acknowlegdement will be sent to: emily,curiel@dss.ca.gov by 1/19/26 Erin Virrueta Emily Curiel DATE: 12/18/2025 DATE: 12/18/2025 Page: 3 of 4 CALIFORNIA DEPARTMENT OF SOCIAL SERVICES COMMUNITY CARE LICENSING DIVISION CHICO CC RO, 520 COHASSET RD., SUITE 170 CHICO, CA 95926 FACILITY NUMBER: 483008401 VISIT DATE: 12/18/2025 Four Staff (S1 - S4) were interviewed on 11/12/25 and stated that C1 and C2 were left behind in the classroom for approximately 3-5 minutes. S4 stated that when they found C1 and C2 and that the children were sitting on the floor playing. S1 stated they walked in and saw C1 and C2 looking at the lights on the costume. No staff stated that the C1 or C2 appeared distressed when they were found in the classroom. Based on information reported and interviews conducted the following deficiency is being cited on the LIC809-D: 101229(a)(1) No child(ren) shall be left without the supervision of a teacher at any time.
