Based on observation, interview and record review, the licensee did not comply with the section cited above in eleven out of eleven children present during public school hours. Where LIC does not have a current private affadavit on file. Which poses a potential health, safety or personal rights risk to persons in care.

LIC will submit proof of private affadavit being granted through CA Board of Education, authorizing active enrollment of 16 children. Additionally, a plan of operations will be submitted to differentiate private and school age programs, including indoor and outdoor space.
Based on observation, the licensee did not comply with the section cited above in two of the stairs to the playground structure were broken at time of inspection. Playground equipment was dusty & had cobwebs on separate areas of the playground structure. This poses a potential health, safety or personal rights risk to persons in care.
