LPA's observations, record review and interviews, Licensees did not prevent child #3 from hurting child #1 during the hours of operation. Licensee was aware of children's challenging behaviors and did not meet the children's need buy not providing close supervision at all times. this poses an immediate risk to children in care.

Licensee will install and utilize camera as needed when stepping away. Licensee shall take a child development training on how to redirect children with challenging behaviors. Licensee shall provide supervision at all times and document on a log that she confirms cameras are working when children are in care and provide a copy of the training to the Palmdale Regional Office by email by the end of the month.

Reporting Requirements
Minor
Interview conducted with licensee. Licensee stated incident occurred 2-2-26 and did not report this to the Palmdale office. UIR is regarding Child #1 hitting Child #2 with the fist on the side of the face causing a bruise. This poses a health and safety to the children in care.
