Based on interview and record review, the licensee did not comply with the section cited above by not having a completed health screening for 3 out of 4 staff present during the inspection which posed a potential health, safety or personal rights risk to persons in care.

Assistant Director, Vega stated that she will complete a health screening as well as Staff #1 (S1), Staff #2 (S2) and submit reciept of its completion to the SDCCRO no later than 10/30/2026. Vega stated that she will send an appointment confirmation if date is passed the due date.
