Based on observation and interview, the licensee did not comply with the section cited above in that there were medications, shampoos, tools, and several hazardous items accessible to children on the indoor and outdoor of the facility which poses a potential health, safety or personal rights risk to persons in care.

Licensee's Assistant Jacqueline Leal immediately removed the medications and hazardous items during time of inspection. Assistant stated that she will ensure that the licensee submits a writtenn plan on how she will ensure that all medications and hazardous items are made inaccessible to children and provide proof to the Department, no later than August 4, 2026.

Licensee’s admission, sometime between April 2021 & June 2021, licensee raised concerns with the responsible party regarding scratches and bite marks on three daycare children in care. Licensee failed to report suspected neglect/abuse to the Dept. This is a potential health & safety risk to children in care.
