Based on observation, interview and record review, the licensee did not comply with the section cited above, in that LPA observed 13 children in the 2 year old classroom being supervised by one teacher which poses a potential, safety, or personal rights risk to persons in care.
A correction plan is recorded. No state verification date is shown.

Facility Representative immediately transferred one daycare child to another classroom. Facility Representative stated that she will conduct a training with all staff and submit a summary of the training and sign in sheet to the Department, no later than June 15, 2026.
