Based on observation, interview, record review, the licensee did not comply with the section cited above in that LPA observed C1 who is 16 months old, sleeping on a cot with a fleece blanket covering their entire head and face which poses an immediate health, safety or personal rights risk to persons in care.

Licensee was provided with a copy of the Title 22 Regulation Section 102425 Infant Safe Sleep. Licensee stated that she will review the regulation, sign and date it and also provide a plan to the Department detailing how she will ensure that the facility is in compliance with infant safe sleep. Licensee stated that she will provide the plan to the Department, no later than May 28, 2026.
