Recorded plan of correction
Facility had a staff member under the preschool license who was not fingerprinted, cleared or associated to the facility. Staff member did not follow up with the application of the criminal record clearance. During today’s inspection on 1/21/26 the staff member left the facility and went to the UPS store to get fingerprinted. Staff cannot return until clearance, facility will email LPA Alvarado the documents of criminal record clearance process and LPA will conduct a follow up inspection.
