Based on observation, interview, and record review, the licensee did not comply with the section cited above in one out of one object, LPA Alvarado observed a blanket in the crib with a napping infant which posed a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above in one out of one person, during the inspection one infant present in the pack N Play managed to get their foot caught under the ripped side of the stitching of the Pack N play where the infant got their right foot caught which posed a potential health, safety or personal rights risk to persons in care.

Licensee and assistant removed the infant immediately, and the licensee removed the crib immediately replacing it with another pack N play making the older one inaccessible to children in care. Licensee fixed it immediately, a follow up inspection will be conducted to ensure that Pack N Play have no tears or are loose on the side.
