Recorded plan of correction
LPA observed staff placing tape over the handles for hot water. Facility had the city maintenance crew arrive quickly, who turned off the water making it inaccessible. LPA was able to confirm during today’s inspection that now the sinks accessible to children do not provide hot water. LPA also provided the facility with the 101239 Fixtures, Furniture, Equipment and Supplies regulations for reference. Ali Zebila Diana Alvarado DATE: 06/16/2026 DATE: 06/16/2026 Page: 3 of 3
