Based on observation, interview, and record review, the licensee did not comply with the section cited above in which the licensee has not provided the parents of the currently enrolled infant in care the LIC 9227 form which poses a potential health, safety or personal rights risk to persons in care.

Licesnee will be provided with a copy of the LIC 9227 form, Licensee will ensure to make copies and provided to families when enrolling infants under 12 Months. Licensee will send via email proof of LIC 9227 for Infant Addie Mae Lee by 6/3/2026. LPA also informed the licensee that a TV was issued on 3/25/25 regarding the LIC 9227.
