Per the facility representative, in the future if staff notice that children are extra active on any particular day, they will radio for the site supervisor’s assistance and to provide an extra staff for support. Ana Chico Veronica Martinez-Garza DATE: 06/08/2026 DATE: 06/08/2026 Page: 4 of 4 CALIFORNIA DEPARTMENT OF SOCIAL SERVICES COMMUNITY CARE LICENSING DIVISION MONTEREY PARK CC RO, 1000 CORPORATE CNTR DR. 200-B MONTEREY PARK, CA 91754 FACILITY NUMBER: 198016764 VISIT DATE: 06/08/2026 LPA conducted interviews with C1 and C2 who corroborated that C1 jumped from the tunnel, landed on their arm/elbow, and called staff for assistance. According to C1 and C2, S2 immediately assisted C1 and provided an ice pack. Per C1 and C2, S3 and S4 were also present during the incident. According to S1, there is always 3 staff present during outdoor play, and staff are good at spreading out to ensure that all children are supervised. LPA advised the facility representative that staff should assess if an incident/accident will require for more than 1 staff to assist and if so, staff should communicate with each other to ensure that the remaining staff provides care and supervision. If additional staff is needed to provide care and supervision, assistance should be radioed immediately. Based on interviews, LPA found that the incident could have been prevented. The following deficiencies were cited in accordance with Title 22 of the California Code of Regulations and Health & Safety Codes. Please see 809D for documentation of deficiencies. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Kenia Villegas.
