This requirement was not met as evidenced by... Based on interviews and records review, the licensee did not comply with the section cited above in that C1 was left unattended in the Toddler yard, which poses/posed an immedate health, safety, or personal rights risk to children in care.

Director shall submit to the Department (seena.parsapour@dss.ca.gov) a written statement explaining how they plan on ensuring compliance with this regulation moving forward, no later than close of business 9/11/2026. LPA observed staff providing visual supervision to children in care during today's inspection. LPA Parsapour will place a referral for this CCC to the Technical Support Program (TSP). An immediate Civil Penalty of $500 was assessed pertaining to this deficiency.

This requirement was not met as evidenced by: Based on record review and interview with Director, the CCC did not comply with the regulation cited above in that one (1) water faucet exceeded the Action Level Exceedance per regulations.

This poses a potential risk to the health and safety of the children in care. LPA observed the fixture located inside a classroom that is locked & not in use by the CCC. LPA also observed on-site correction as an "out-of-order" sign was affixed to the affected faucet. Director shall notify parents of children enrolled and shall continue providing bottled water to children in care. Director shall submit a written plan of correction no later than 5/25/2026.
