Recorded plan of correction
Director stated that since the incident occured, the facility schedule has been modified to reduce the number of transitions during the day and the staff responsible for supervising C1 was fired. This requirement was not met as evidenced by: the Director UIR on 08/25/21 stated C1 was left without supervision of a teacher for 3-4 minutes when children were playing in the playground. This presents a potential threat to the health and safety of children in care.

1. Have all staff meeting about procedures and policies of Supervision. Staff meetings are every wed morning.
2. Installed second latch on gate.
3. Meeting with Parent of C1 which was 08/27/2021.
4. Inform all Parents of what happened in newsletter.
Notes of the staff meeting (09/01/2021) will be emailed to LPA Trinh by 02/03/2022.
