Based on (observation) & (record review), the licensee did not comply with the section cited above in (1) infant child did not have an infant safe sleep log on file which poses a potential health, safety or personal rights risk to persons in care.

Licensee will begin documenting the infant safe sleep log the next time the infant falls asleep and will provide LPA a copy of infant safe sleep to LPAs email Fernando.Hernandez@dss.ca.gov
