Based on interviews on 02/02/2026, Licensee self-admitted that on 01/27/2026, a child was found unattended without an adult supervision on a step of the door of the home, outside of the day care home and staff did not provide adequate supervision to the day care child, which poses an immediate Health, Safety, or Personal Rights risk to persons in care.

By plan of correction due date, 03/10/2026, Licensee will submit the written statement of understanding of the Title 22 required regulations of licensee shall ensure that children in care are supervised at all times by an adult. Licensee has a self latched gate on the side of the backyard. Licensee stated that door to the offlimit areas of the home is always locked during hours of operation. Immediate Civil penalties of $500 assessed today.
