Based on results received from the certified water samplers, the facility exceeded 5.4 ppb of lead in their water source. This posed a potential health, safety, or personal rights risk to persons in care.
A correction plan is recorded. No state verification date is shown.

The facility replaced the fixture and retested the new fixture, which tested below the threshold for an ALE. Director notified LPA of test results for new fixture on 3/14/23. Deficiency will be issued and cleared on the same day of visit.
