Based on record review, LPA observed that many children's immunizations were not transferred onto the CA School Immunization Record form (CDPH 286), which poses a potential health, safety or personal rights risk to persons in care.

Licensee shall ensure she transfers all children's immunization records onto the CA School Immunization Record form (CDPH 286) and submit copies of all children's forms to LPA by due date of 3/27/26. ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
