      Outdoor Activity Space

      Based on observation, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. The rope ladder to on the Large Lifetime play structure & all of the geometric shaped climbing play structure are not properly secured to the ground.

      Plan of correction: Licensee has immediately made these two items off-limits to all children and staff. Licensee will have the items either secured to the ground or removed from the outdoor play area entirely no later than 01/07/2022. Once the items are secured, licensee will notify LPA Uribe and LPA will come out to inspect the correction and clear the deficiency. Children are not to play on the structure until after LPA Uribe has come to inspect the structures.

      Child's Records

      Based on record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA Uribe observed that out of the 10 children's files reviewed, 9 of them had at least one incomplete licensing form. LPA Uribe discussed in depth which child was missing what information on which forms with the licensee.

      Plan of correction: Licensee will thoroughly inspect all children's files and carefully read each form to ensure that all are filled out properly and correctly. Licensee will also correct the missing information on the children's forms which were reveiwed during today's inspection no later than 01/07/2022.
