LPA observed a trampoline that had frayed material and broken plastic on the bottom that is a potential hazard for children. A slide from a nearby play structure was propped up to slide into the trampoline opening and not part of the manufacture's recommendations which poses/posed a potential health, safety or personal rights risk to persons in care.
