      Administration of Child Day Care Licensing

      Based on record review, the licensee did not comply with the section cited above in Staff (S5) was missing vaild mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Site Director will have S5 complete mandated reporter training and send it to the department by POC due date 04/10/2026.

      Lead Testing

      Based on interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Director stated that the Cambrian School District conducted water lead testing but did not have a copy of the report to provide to LPA and was not sure if the water faucets in their preschool were tested as part of the School District testing.

      Plan of correction: Director will obtain testing report from the School Distrist or complete water lead testing and submit report along with the required forms to LPA by 04/05/2024.

      General Provisions and Definitions

      Based on observation, interview and record review, the licensee did not comply with the section cited above for two staff, S1 and S2 which poses a potential health, safety or personal rights risk to persons in care. Staff S1 is missing proof of immunizations for MMR and Tdap and S2 is missing immunization proof for pertussis.

      Plan of correction: Director will have staff complete immunizations and submit proof to LPA by plan of correction date of 04/05/2024.
