Based on interview, the licensee did not comply with the section cited above. The director failed to report an unusual incident to the Regional Office, which poses/posed a potential health, safety, or personal rights risk to children in care.
A correction plan is recorded. No state verification date is shown.

The director will ensure all staff are retrained on reporting requirements. Staff will be required to watch the reporting requirements video available on the CCLD website, and the director will provide a log documenting each staff member’s completion of the training.
