During the facility visit, LPA conducted site observations and interviewed D1. Based on record review and interview, staff adhered to proper emergency protocols and medical procedures. The facility also complied with all required reporting requirements. D1 has since reviewed safety protocols with teachers to help prevent similar incidents from occurring.
LPA discussed with D1 the playground’s age-rating and D1 stated they understood and will make different arrangements for field trips. A Type B deficiency will be issued regarding age-appropriate play equipment. A plan of correction was discussed and agreed upon. Appeal rights were provided and explained.
A notice of site visit was provided and must remain posted for 30 days.
