Waivers and Exceptions for Program Flexibility
Minor
Based on records reviewed and interviews conducted, it was found that the center did not comply with its approved licensing playground waiver for McCoppin Square Park. The waiver requires that preschool children be escorted to and from the park by adult staff and that a ratio of one adult to every six preschool children be maintained during transit and while using the playground. On the date of the incident, two teachers escorted thirteen preschool children to the park, which did not meet the required one to six adult to child ratio. Failure to follow the approved waiver conditions poses/posed a potential health, safety or personal rights risk to persons in care.
A correction plan is recorded. No state verification date is shown.
Recorded plan of correction
The director stated that the center will conduct a staff meeting and provide refresher training on the approved licensing waiver requirements, including escort procedures and the one to six adult to child ratio during transit and while at the park. The director will submit the training agenda, training materials, and teacher signatures of those who attended for LPA review by the due date.
