This requirement was not met by: Based on iterviews, the facility did not inform SDRO that S1 bounced a doll on C1’s head. The previous Director acknowledged to SDRO that the incident was not reported within the required timeframe.

The facility will conduct an all staff meeting to review procedures for reporting incidents to the Director and for notifying SDRO directly of any unusual incidents that occur. The Director will send the SDRO a staff sign in training sheet by 10/9/26.
