Based on observations, interviews and records reviewed, the facility commingled infants with preschool children. Infants were napping in the preschool classrooms #3 and #4 which poses a potential risk to the health and safety of those in care.

The facility representative, Kinda Sancha, stated that effective immediately, infants will no longer be placed in preschool classrooms. Based upon staff and Director interviews, the front gate was not working properly which is a potential health, safety and personal rights risk to children in care.
