Analyst provided licensee with a sample safe sleep log and she states she will conduct 15 minute observations and log them every time the infant she has in care is present and naps until he wakes up through the age of two years and will continue to do so with every new infant she enrolls. Based on analyst interviews, the licensee did not comply with the section cited above as she was found to have left children unsupervised on multiple occasions in an open gated front yard which is an immediate risk to their health and safety.

Licensee states she will draft a plan to ensure that children are supervised at all times and submit it to analyst for his review, by 09/07/23, to complete the correction. Licensee states she will also watch "Supervising Children in Family Child Care" video on CCL website and provide written confirmation of viewing and summary of video content and submit it to analyst by same date.

Based on analyst observation, interview and record review, the licensee did not comply with the section cited above as assistant, Josette Saintilus, has been working at the facility without a final criminal record clearance which poses/posed an immediate health, safety or personal rights risk to children in care.

LPA discussed the following with licensee: that all new adults living or working in the home must have criminal record clearance or exemptions and child abuse index clearances prior to employment or residing in the home. Licensee acknowledges understanding of requirement. While Ms. Saintilus was Livescan fingerprinted, her clearances remain in pending status and she is not allowed to maintain employment until her final clearances have been received. Ms. Saintilus was to be taken home by Mr. Milord and may return when her clearances have been completed.

Based on analyst observation, the licensee did not comply with the section cited above when a fist sized hole was found in the wall behind the home back door and a large screen television was sitting unsecured on two chairs in a for use bedroom which poses/posed a potential health, safety or personal rights risk to children in care.

Licensee spouse Marc Milord states he will address both items by sealing the holes in the wall and installing a stopper at the top of door to prevent the doorknob from making new holes in the wall if slammed and by mounting the television in a secure bracket on the wall. In the meantime, licensee and spouse will use increased supervision to keep children from accessing the wall by the back door and keeping the bedroom off limits to day care children until the television repair is completed. Analyst will conduct an unannounced return visit to review the corrections.
