Based on LPA's observations, records review, and interviews, the Facility did not comply with the section cited above as there were rat feces found in the facility. This poses a potential health, safety or personal rights risk to persons in care.

Administrator states the facility will maintain a cleaning protocol of daily cleaning and also deep cleaning. LPA reviewed the Building and Grounds and Infant care General Sanitation regulations with the Administrator. Administrator states facility will send the San Diego Regional Office a written plan of correction on the new cleaning protocol plan and proof of continued pest control services by 12/9/22.

The director also had a training meeting with staff regarding the facility’s new steps releasing children from care. The director stated a written plan of correction describing new procedures on supervision and steps on releasing children from the facility and training agenda including staff attendance sheets will be submitted to the Department by 06/17/2022.
