Based on the self-reported incident report and interview with Site Director Anne Patzold, 2 preschool children were left behind the enclosed playground without adult supervision for approximately 13 minutes. This is a repeat violation.

Ms. Patzold stated that all 3 staff who were in charge of the room that day were dismissed immediately. She is scheduled to meet with her staff tonight 11/30/2022. Part of her agenda is to discuss visual supervision of children at all times. Ms. Patzold shall submit a copy of the agenda and a sign-in sheet via email no later than 12/1/2022. This requirement is not met as evidence that on 09/01/2022, a two year old child was left behind at the enclosed playground around 8:18am while the rest of the children and three staff members returned back into their classroom. The child was brought back into her classroom approximately six mintues later at 8:24am. This poses a potential health and safety risk to clients in care.
