The Director provided a copy of the training agenda and staff sign in sheet for the staff training held on 10/11/21. The director will submit a signed and dated statement explaining how she will ensure that staff are following the correct procedures for transitions/supervision. The director will also provide verification that management is monitoring how staff are implementing supervision/transition procedures to LPA via email by POC due date.
