Based on record review, the licensee did not comply with the section cited above in that Staff #3 did not have a health screening on file and the TB test on file was dated over 12 months prior to hire. This poses a potential health, safety or personal rights risk to children in care.

Based on LPA observation, record review and interviews with staff, parents and children, staff did not maintain visual supervision. This resulted in inappropriate interactions between children in their classroom. This poses a potential health and safety risk to children in care.
