Staff 1 (S1) stated that C1 was upset and didn’t want to put sunscreen on, so they rode their scooter away. S1 followed C1 when they rode through the basketball courts on campus, the middle school beside the campus, and across the street into a neighborhood. By the time that S1 was at the corner of Viejo and Sage Street, they had lost sight of C1. S1 was in contact with the police and the police conducted a wellness check on C1 and spoke to their grandmother. S1 was informed that C1 was in their home with their grandmother. The police then told S1 to return to the facility.
Staff 2 stated that they assisted S1 as C1 left the facility. S2 was informed by C1's authorized representative that C1 may have gone to their grandparent's house. S2 drove to the neighborhood where C1 was when C1 was already in the home and the police had already gone to the home.
S1 took LPA on the path that C1 had taken when they left campus. It was determined that S1 had lost sight of C1 approximately 450 feet away from their home where they were found by the police.
In the areas that were evaluated, the facility was not in compliance and is in violation of the California Code of Regulations, Section 101229(a)(1). The deficiency is being cited on the attached LIC 809D.
