Supervision and child counts
Minor
Based on LPA observation, D1 did not comply with the section cited above when 8 children and D1 were in an enclosed playyard. C6 opened the gate by themself, ran to a secondary gate (which lead to a side street) then exited by themself. After gathering the remaining 7 children, which took a few seconds, D1 was able to run after C6 with the remaining 7 children in tow. LPA observed C6 on the otherside of the street with D1 trying to cox him back to the facility side of the street. A few minutes passed until C6 willingly rejoined the group. No cars were observed to pass through. This poses a potential health, safety or personal rights risk to persons in care.
A correction plan is recorded. No state verification date is shown.
Recorded plan of correction
D1 stated that she will talk to parents about todays incident and provide a UIR report with in 7 days. D1 will create a plan with parents to create a support system for C6's behavior. D1 will replace the gate and/or locking mechanism on the infant playyard so that children are not able to open the gate themselves. D1 will provide a copy of the plan and a picture to jennifer.patel@dss.ca.gov.
