Supervision shall include visual observation. This requirement was not met as evidenced by a preschool child who was left in the calassroom with no adult supervision during transition time for indoor play in the gym. This is an immediate health and safety risk to the children in care.
A correction plan is recorded. No state verification date is shown.
Recorded plan of correction
The director states management staff will be observing all the transitions for the next 30 days and also checking the child supervision record at least twice a day for accuracy. A staff training is scheduled for July 30, 2026 that will discuss supervision. The director agreed to email LPA L. Habtom a meeting agenda and the sign in sheet to clear the deficiency.
