Based on observation, interview, record review, the licensee did not comply with the section cited above in that Staff #4 and Staff #6 were present and working prior to being fingerprint cleared, which poses an immediate health, safety or personal rights risk to persons in care.

Staff #4 and Staff #6 left the facility. Director submitted a correspondence to LPA Tillery stating that she understands that staff are to be live scanned, fingerprint cleared and associated to the facility prior to working.
