Based on observation, record review, and interview the licensee did not operate within limitations specified on the license by commingling pre-school and school age children. This poses an immediate health and safety risk to persons in care.

report from 05/19/2023. On 05/19/2023, a former staff physically removed a children from a plastice seat after the child did not listen to the teachers repeated instructions to sit on the seat properly. This was considered a potential threat to the health and safety of the child at the time.
