Criticalreported to LPA via email on 12/21/22. On 12/15/22, Child #1 was found with a vape pen in her mouth by Staff #1 and Staff #2, after it was left on the playground from Staff #3. Evidence was also conclusive that Staff #3 previously used the vape pen in the staff break room on site. This posed an immediate risk to health and safety of children in care.

Staff #3 is no longer employed at the facility. Director stated that she will review with staff procedures for safety checks of the playground before usage by children. Director will reiterate to staff that smoking is not allowed on or near the premises.
