Critical
Supervision shall include visual observation. This requirement was not med as evidenced by admission from the facility through self-reporting, staff interviews, and review of video footage. On 09/16/26, Child 1 was left unattended in the Busy Bee classroom for approximately 8 minutes, before they were observed by Staff 1. This is considered and immediate risk to the health and safety of children in care if not corrected.

Facility representive, Cassandra Spell, stated that they will hold an all-employee training to go over procedures for transitions. Company will re-intstate the name to face online training. During today's visit, a civil penalty will be assesed. Facility representative will send proof of staff completion of the training by 9/24/26.
