      Based on record review, the licensee did not comply with the section cited above as Staff 1 CDA certification expired 07/2025. Staff 1 did have proof of completion of Child, Family, and Community nor Infant and Toddler Units, which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Staff 1 will recertify their CDA or enroll to complete the required classes (Child, Family, and Community and Infant/Toddler Development) and send proof to LPA by the POC Due date 06/26/2026.
