Based on the Center Director’s admission, corroborating staff interviews, and documentation reviewed during the investigation, it was corroborated that Staff S2 violated children’s personal rights by using food and mealtime as a form of discipline. Specifically, Staff S2 withheld food from children as a consequence for their behavior, threatened children with consequences if they did not behave, and discarded a child’s food after the child did not comply with S2’s directions. This poses a potential health and safety risk to children in care.
A correction plan is recorded. No state verification date is shown.
Recorded plan of correction
The Director will provide S2 training on Title 22, Section 101223, children’s personal rights, and appropriate behavior-management practices, including the prohibition against using food as discipline. Additionally, S2 will view CCLD's training video on "Children's Personal Rights in Child Care"at https://ccld.childcarevideos.org/child-care-center-operators/childrens-personal-rights-in-child-care/ Training will be documented. The Director will conduct and document ongoing observations of S2 to ensure compliance and appropriate interactions with children. Documentation of the training and monitoring will be submitted to CCLD by the POC due date as proof of correction.
Based on the Director’s admissions on May 13 and May 27, 2026, corroborating staff interviews, and documentation reviewed during the investigation, the LPA determined that staff assigned to function as teachers at the facility did not meet the minimum teacher qualification requirements pursuant to CCR Section 101216.1. Specifically, staff were assigned teacher duties without completing the required educational coursework or obtaining an applicable permit prior to employment as a teacher. This poses a potential risk to the health and safety of children in care.
A correction plan is recorded. No state verification date is shown.
Recorded plan of correction
By the POC due date, the Director will review all preschool personnel files, verify that each teacher meets the educational qualifications required by CCR Section 101416.2(b), maintain transcripts in each personnel file, and ensure that any staff who do not meet the qualifications are not assigned infant care teacher duties. The Director will submit to CCLD a current staff assignment or classroom roster identifying each staff member's assigned position, demonstrating that any staff who do not meet the qualifications are assigned only to positions for which they are qualified.

Based on the LPA's observation on 11/14/25 and 11/20/25, the LPA observed observed that the facility’s Little Tikes plastic play structure was not in safe operating condition.
