met as evidenced by a self-reported incident that occurred on 12/2/2025. Child 1 was left unattended in a classroom for approximately 1 minute before they were found by staff. This posed an immediate risk to the health and safety of children in care.

All staff have completed training on name to face training. All staff must now call an admin via walkie talkie when transitioning children to and from their classroom. Minor
by the unusual incident report, which was reported to LPA on 10/03/2023. Child #1 was left in the bathroom on the playground with the door closed for approximately 1 minute. The child was not injured. This is a potential risk to the health and safety of children in care.
A correction plan is recorded. No state verification date is shown.

Staff was placed on aministrative leave in order for the facility to conduct an investigation. Staff were retrained on supervision. Staff now follow new procedures when children have to use the bathroom while using the playground.
