ridicule, coercion, threat, mental abuse or other actions of a punitive nature ... This requirement was not met as evidenced by interviews, video, and other documented evidenced. On 03/06/2026, Staff 1 threw toy blocks at a child in care as a response to their actions. Staff 1 also used inappropriate language in the prescene of children in care during a verbal altercation with a parent. This posed an immediate threat to the health and safety of children in care.

Staff 1 was placed on leave as of 03/06/2026. Staff 1 is still under investigation with the company. A decision regarding their employment will be made when the investigation is finished. All staff currently employed at the facility will complete personal rights training. A description of the training and signatures from staff who completed the training will be sent to LPA by 04/10/2026.

Supervision shall include visual observation. This requirement was not met as evidenced by video and interviews. On 03/06/2026, Child 1 was able to leave the facility and climb over a fence located near the facility without the supervision of staff. This posed an immediate risk to the health and safety of a child in care.
