Based on record review and interview, the licensee did not comply with the section cited above in that 3 out of 4 staff records (Staff 1 [S1], Staff 2 [S2], & Staff 3 [S3]) did not have a Health Screening available to review. Additionally, 1 out of 4 staff (S2) did not have proof of their immunity to Measles available to review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Facility will submit proof of the Health Screening (including tuberculosis test according to California Code of Regulations (CCR) Section 101216(g)(1)) for S1, S2 and S3. Additionally, the facility will submit proof (copies) of proof of immunity to measles for S2. The facility will submit these items to the Department by the POC due date.
Based on records review of required lead testing, the facility had lead values of 5.5 or above on water outlets: Outlet A (68 ppb), Outlet B (5.6 ppb), Outlet C (6.5 ppb) and Outley E30 (8.2 ppb). This is a potential health and safety risk to persons in care.

Facility will implement corrective action pursuant to section CCR 101704 for immediate cessation of outlets testing with action level exceedance until it is replaced and retested pursuant to section CCR 101705 and returns a result at or below the Acton level. Facility will notify CDSS with completion within 30 days. Facility is currently waiting on getting a water filtration system installed and will be retesting the water outlets for future use.
